Last date for receipt of application form: 27.07.2009 at 5.00 pm (Monday)
Date and time of Entrance Examination: 09.08.2009 (Sunday) 10.00 AM to 01.00 PM
Venue/Centre: Rohtak
The prospectus containing eligibility conditions, domicile criteria, application form and OMR form can be obtained from the Asstt. Administrative Officer (Acad.), Pt. B.D. Sharma University of Health Sciences. Rohtak on cash payment of Rs. 500/- (Rs. 125/- for SC/BC of Haryana only) w.e.f 07.07.2009. Request for prospectus through Registered Post must be received in the office of Asstt. Registrar (Academic) on or before 20.07.2009 by sending a crossed bank Draft of Rs. 550/- (Rs. 175/- for SC/BC of Haryana only) drawn in favour of Controller of Finance, Pt. B.D Sharma University of Health Sciences, Rohtak payable at State Bank of India, Pt. B.D Sharma P.G.I.M.S, Rohtak (Code 4735) failing which the request will not be entertained.
The Application Form complete in all respects must reach the Controller of Examination, Pt, B.D Sharma University of Health Sciences, Rohtak by 27.07.2009 upto 5.00 PM. The application received late shall be rejected.
