The Information Bulletin containing Application Form can be obtained against cash payment of Rs.800/- for General/OBC Category and Rs. 450/- for Scheduled Caste/Scheduled Tribe Category from any of 177 specified branches of Canara Bank situated in the different part of the country. The list of Banks is given in Appendix IX. Fee once remitted shall not be refunded under any circumstances.
